    Provides information and guidelines on registration (e.g., requirements, application process, and operation screening).

    Contains examination schedule and fees, requirements of registration, a 9-page application form, and examination location preference form.

     

  • Application Package Cover Letter
    Contains the cover letter for applicants to the registration process.

     

  • Examination Prep Guide
    Provides typical examination questions (and answers, suggested study outline and material.

     

  • Problem Solving in Preparation for the NRRPT Exam Order Form
    Contains the order form for the book.  The objective of this book is to guide the reader toward a mastery of working radiation protection problems at the technologist level.

     

  • Examination Retake Request
    Short application form for individual who have previously been approved to sit for the examination.

     

  • Candidate Subscriber Application
    This program was developed for individuals who do not yet have the required five years of experience to apply for the examination but desire Registry information. The cost is $10 per year and the subscriber receives the registry Handbook and Newsletter.

 

 

 

  • Sponsorship Forms
    Contains the forms to apply for each sponsorship level.

     

  • Registration Maintenance Program
    Contains the entire registration maintenance program documentation.

     

  • Registration Maintenance Worksheet
    Contains the worksheet that is used to document your credits.

     

  • Plaque Order Form
    Use this form to order your plaque. Complete the form and mail it to the Registry Office

     

  • Scholarship Application
    Contains an application for the NRRPT Scholarship, awarded to active registered members who are planning on attending an accredited educational institution in a course of study in Health Physics or a closely related field.

     

  • Sustaining Membership Notice
    Contains the form to use to submit your membership fees.

     

  • Transcript Request
    This form requests that the registry directly notify a college or university that the member successfully passed the examination and should be awarded the recommended college credits.
